Being prepared can save you a return trip. Here is what you may need depending on the reason for your visit:

💰 For Retirement Benefits

  • Social Security card or number
  • Birth certificate or proof of age
  • W-2 forms and/or self-employment tax returns for the last year
  • Bank account information for direct deposit (routing and account number)
  • Proof of U.S. citizenship or lawful alien status (if not born in the U.S.)
  • Military discharge papers (DD-214) if you served before 1968

📋 For Disability Benefits (SSDI/SSI)

  • Social Security card or number
  • Medical records, doctor names, addresses, and phone numbers
  • List of all medications with dosages
  • Work history for the past 15 years (job titles, duties, dates)
  • Most recent W-2 or tax return
  • Bank account information for direct deposit

💳 For a New or Replacement Social Security Card

  • Proof of identity (unexpired driver's license, state ID, or U.S. passport)
  • Proof of U.S. citizenship (birth certificate or U.S. passport)
  • Proof of immigration status (if applicable)

  • Form CMS-L564 (Request for Employment Information) if enrolling during a Special Enrollment Period

💜 For Survivor Benefits

  • Deceased person's Social Security number
  • Death certificate
