What to Bring to Your Appointment
Being prepared can save you a return trip. Here is what you may need depending on the reason for your visit:
💰 For Retirement Benefits
- Social Security card or number
- Birth certificate or proof of age
- W-2 forms and/or self-employment tax returns for the last year
- Bank account information for direct deposit (routing and account number)
- Proof of U.S. citizenship or lawful alien status (if not born in the U.S.)
- Military discharge papers (DD-214) if you served before 1968
📋 For Disability Benefits (SSDI/SSI)
- Social Security card or number
- Medical records, doctor names, addresses, and phone numbers
- List of all medications with dosages
- Work history for the past 15 years (job titles, duties, dates)
- Most recent W-2 or tax return
- Bank account information for direct deposit
💳 For a New or Replacement Social Security Card
- Proof of identity (unexpired driver's license, state ID, or U.S. passport)
- Proof of U.S. citizenship (birth certificate or U.S. passport)
- Proof of immigration status (if applicable)
- Proof of name change (if applicable: marriage certificate, divorce decree, or court order)
🏥 For Medicare Enrollment
- Social Security card or number
- Current health insurance information
- Information about any employer group health coverage
- Form CMS-L564 (Request for Employment Information) if enrolling during a Special Enrollment Period
💜 For Survivor Benefits
- Deceased person's Social Security number
- Death certificate
- Your Social Security card or number
- Your birth certificate
- Marriage certificate (if applying as a surviving spouse)
- Deceased's most recent W-2 or tax return
- Bank account information for direct deposit
